HIPAA note

This form is not a clinical record. Clinical records live in the practice's HIPAA-compliant client portal. Please do not submit protected health information through this form.

Your choices

You may request deletion of any pre-clinical information you submitted by emailing julie@inflectiontherapyandconsulting.com.
